CareCredit Policy

For patients using CareCredit:

  • You must first apply for CareCredit and receive an approval confirmation number.
  • Once approved, you may schedule your appointment using a $500 refundable deposit to hold your spot.
  • This $500 deposit is paid separately and is not processed through CareCredit.

Once full payment is received through CareCredit (30 days prior to your surgery), your $500 deposit will be refunded.

Please note:

  • The $500 deposit option is strictly reserved for patients who have been approved for and intend to use CareCredit.
  • Patients not using CareCredit must pay the full amount at the time of scheduling.
  • If CareCredit is not used for full payment within the required timeframe, the $500 deposit may be forfeited.
  • The $500 deposit also becomes nonrefundable according to our Cancellation Policy.

Insurance Policy

Vasectomy reversal procedures are cash-pay services and are not billed through insurance.

These services are provided through our affiliated entity, Laboratory and Procedural Services (LPS), which allows us to offer some of the most competitive pricing in the United States.

Cancellation Policy

Surgery must be cancelled at least 2 weeks before the date of surgery to qualify for a full refund (click here for full policy).
